   The Empire Plan offers a personalized web site for subscribers at
www.myuhc.com. Here you can review your claims history and print out copies of your Explanation of Benefits forms. Registration is required. When registering enter #30500 in the Group/Account field.

Medco by Mail (mail order prescriptions)
   Medco by Mail is now the Empire Plan's mail order prescription service for new prescriptions as well as for renewals of prescriptions placed with Caremark.

403(b) Tax-deferred Annuities
   The OMNI Group, the company that the district has employed to administer its 403(b) plan, maintains an active web site at 
www.omni403b.com. Once there click on “Clients”/ “Client Home Pages” and then select “Long Beach City SD (NY)” from the list of organization’s home pages. Here you can download forms, find answers to frequently asked questions and review the list of participating providers.

NYSUT Member Benefits
The NYSUT Member Benefits Trust was established in 1983 as a tax-exempt, not-for-profit trust distinct from NYSUT for the purpose of endorsing and monitoring quality, competitive insurance plans and benefit programs. These programs and services are available to NYSUT members and agency fee payers. Some are also available to or cover spouses/domestic partners of members, dependent children, dependent parents and grandparents.

Eyeglass Reimbursement Members have $400 to spend on glasses/contacts. To better accommodate the purchase of over the counter glasses, the district has created a new claim form which should be available in all of the main offices. On the form you will attest that the glasses are for your personal use. Completed claim forms with receipts attached must be sent to the HR department c/o Lido School. Eyeglass Reimbursement Form

EAP
   The Employee Assistance Program is available to assist our members with just about any problem that may arise. It is open 24 hours a day/7 days a week.

Cancer Screening
Employees of the District may be excused from their duties for a sufficient period of time, not to exceed four hours on an annual basis (July 1-June 30) specifically for breast and prostate cancer screening. Please be sure to follow district procedures for notification of your absence so that appropriate coverage may be secured. Upon return from your screening leave, you must complete the
Cancer Screening Verification Form. The original must then be submitted to the Office of Human Resources. The leave will not be charged to any bank.

   The Flex Plan (a.k.a. Cafeteria Plan) offers our members significant tax savings on their excess medical and child/elder care expenses.  
Preferred maintains an active website at www.thepreferredgroup.com. Here you can download a claim form and find information on what expenses are eligible for reimbursement. Registration is now required to access the site.
